The ElderServe Long-Term Home Health Care Program is designed for persons who choose to remain in their own home although eligible for placement in a long-term care facility. The program provides a full range of health care, social and supportive services, including medical management, nursing services, rehabilitation therapies and personal care in a client's own home. Additional care options may include housekeeping, home maintenance, Adult Day Services and the Personal Emergency Response System.
Care Management
A Case Manager, who is a Registered Nurse, plans, supervises and coordinates all Long-Term Home Health Care services. When a referral is made, a nurse visits the home to conduct a comprehensive assessment of the client's needs. the plan of care is developed in conjunction with the client, their physician and family. Acting as a liaison with the client's physician, and all other ElderServe community programs, the Case Manager responds to all client requests, questions and concerns and reports frequently on the client's status and progress.
