ElderServe Care Managers ensure that each client receives the right combination of services to maintain their health and well being in their own home.
Care Managers assess clients’ needs and formulate a care plan with the clients, their family and physician, if indicated. They initiate referrals to appropriate services and programs and provide ongoing supervision and monitoring of services and the personnel providing these services. Care Managers ensure that services are integrated to best meet client needs and coordinated to maximize quality of life. A Care Manager will address client and family concerns and act as a liaison to all ElderServe and community programs.
Geriatric Care Managers are available on a private basis for both local and out of town families to provide consultation, information and referral, assistance with client relocation, ongoing coordination and supervision of care.
