Geriatric Care Management from the Hebrew Home at Riverdale

To meet the growing demand of aging individuals who wish to remain home the Hebrew Home at Riverdale has designed a Geriatric Care Management program promoting our mission to support older adults in their journey to age well. At the Hebrew Home we understand that aging is accompanied by a host of changes that threaten an individual’s safety and independence. Adult children and spouses are often faced with difficult decisions when navigating these emotionally challenging times. Our Care Management program coordinates a wide range of services so that older adults can remain safely in their own homes.
 

What is a Care Manager?  

The Hebrew Home’s care managers are certified professionals knowledgeable about the resources available to clients, and experienced at navigating the complexities of health care systems. Our experienced care management team focuses on the details of coordinating care so that you can focus on the emotional well-being of your family. We will help plan, implement, and oversee an individual care plan for your loved one.

Care teams consist of licensed clinical social workers and certified geriatric care managers who assess and coordinate specialized in-home services with doctors, nurses, physical therapists, occupational therapist, massage therapist, dietitians, beauticians, pharmacists, medical equipment providers, and trained home health aides. Since it is always difficult to let someone new into your home our services include reference and background checks upon request. Our care management team has taken a Pledge of Ethics in affiliation with the National Association of Professional Geriatric Care Managers.
 

What Services Are Offered?  

A comprehensive assessment includes an evaluation of the physical, psychological, and environmental needs of the individual followed by a detailed care plan. Family and support systems are seen as a vital ingredient to coordinating care, and our care management team prides itself in communicating timely updates regarding any changes in clients functioning. Weekly reports and family consults are available as needed. 



Services are provided on a fee-for services basis and include the following:

  • Hospital Visits and Discharge Planning to transition client safely home
  •  Environmental Home Safety Assessments to identify potential problems and provide solutions such as removing loose area rugs, installing grab bars in bathrooms, etc.
  • Home Health Aide Hiring and Supervision to improve and monitor the quality of in- home attendants and nurses
  • Home Health Monitoring of medications and chronic healthcare issues using innovative technology such as telemedicine when appropriate
  • Coordinate Follow Up Care by arranging transportation, accompanying clients to appointments, refilling medications, tracking future appointments, providing support and education, and facilitating communication among doctors
  • Relocation Assistance in arranging transitions either within care facilities in New York state or elsewhere
  • Referral Recommendations to navigate community resources and promote informed care decisions by family members
  • Long Term Care Planning to ensure that your family has both a plan and means to address future challenges
  • Crisis Intervention during emergency situations 24 hours a day, 7 days a week
  • Counseling and Support for both overwhelmed caregivers and older adults coping with life changes
  • Advocacy in completing complex applications for resources and entitlement benefits
  • Bill Organization and Payment to help a client maintain independent living
