ElderServe offers Social and Medical Adult Day Programs for older people who are either physically frail, lonely, or are experiencing memory loss, Alzheimer's Disease or other dementias.
Social Adult Day Programs
Social day programs offer a full day of social, recreational and educational programs for people who are living at home and might otherwise be socially isolated. Participants enjoy a rich curriculum of stimulating activities that promote socialization and foster self-esteem in a comfortable and safe environment. Assistance is provided for individuals needing help with activities of daily living such as eating, walking or using bathroom facilities.

Social and recreational programming includes educational and health lectures, exercise, intergenerational, horticulture, arts and crafts programs, musical performances, reminiscence groups, holiday celebrations, parties, entertainment and trips.
A continental breakfast and hot lunch are served. Social Adult Day Programs are open five days a week. Participants may attend the program in any combination of days. Round trip, door-to-door transportation is available in comfortable, handicapped accessible vans. The program is paid for privately or by Medicaid through the Long Term Home Health Care Program.

Medical Adult Day Programs
ElderServe on the Palisades is a medical model adult day program, open seven days a week, 365 days a year. The program services seniors with Alzheimer’s and related dementias, those who are physically frail or who have mild to complex medical needs.
We offer transportation, meals, medication management and a range of therapies, including physical, occupational and speech, combined with wonderfully stimulating activities indoors and outside on our 19-acre waterfront campus. Please call ElderServe on the Palisades at 718-581-1613
