For people who wish to stay in the comfort and security of their own homes, ElderServe, the Hebrew Home’s community services division, provides a wide range of health care and supportive services that can enhance and strengthen the quality of life they enjoy. ElderServe offers rehabilitation services, personal care, medical and social adult day programs and a respite program. Caregiver support groups, counseling, information and referrals to community health and legal resources are offered for family members as well. Services are available for people throughout the Bronx, Manhattan and Westchester.

ElderServe Licensed Home Care Services Agencies provide individualized in-home services to people in their own homes. The goal is to ensure the independence and dignity of older people who choose to remain in the community, in their own homes. By developing a comprehensive customized plan of care, people are able to live in the secure environment where they are most familiar and comfortable.

ElderServe Long-Term Health Care Program
The ElderServe Long-Term Home Health Care Program is designed for persons who choose to remain in their own home although are eligible for placement in a long term-care facility. The program provides a full range of health care, social and supportive services, including medical management, nursing services, rehabilitation therapies and personal care in a client’s own home. Additional care options include home maintenance, Adult Day services and a personal emergency response system.
A case manager, who is a registered nurse, plans, supervises and coordinates all Long-Term Home Health Care services. When a referral is made, a nurse visits the home to conduct a comprehensive assessment of the client’s needs and then develops a plan of care in conjunction with the client, the client’s physician and family Acting as a liaison with the client’s physician and all other ElderServe community programs, the case manager responds to all client requests, questions and concerns.
