Home Care
The ElderServe Licensed or Long-Term Home Health Care Program provides a full range of health care, social and other supportive services to help seniors maximize their independence while living in the comfort of their own homes. By developing a comprehensive customized plan of care, an individual is able to live in the secure environment where they feel most comfortable. We service thousands of seniors in Manhattan, the Bronx and Westchester counties.
The ElderServe home care team assists with the instrumental and personal activities of daily living that enable clients to live independently in the community. Staff is carefully screened, fully certi.ed and bonded, and participate in continuing education to expand and enhance their skills.

Case Management
A Case Manager, who is a Registered Nurse, plans, supervises and coordinates all Long-Term Home Health Care services. When a referral is made, a nurse visits the home to conduct a comprehensive assessment of the client’s needs. The plan of care is developed in conjunction with the client, their physician and family. Acting as a liaison with the client’s physician, and all other ElderServe community programs, the Case Manager responds to all client requests, questions and concerns and reports frequently on the client’s status and progress. Long-Term Home Health Care staff is available 24 hours a day, 7 days a week.
