What can I expect from the first visit?
The first visit can be done in a variety of settings but is most often done in either a hospital setting prior to a client’s discharge home or in the client’s own home. The visit typically lasts for two to three hours, and includes input from family members and health care providers. After the initial visit the family will be sent a comprehensive assessment with a prioritized list of recommendations. At the family’s request these recommendations can be carried out with support from one of our experienced geriatric care managers.

How long will I need care management services?
The length of time that an individual utilizes services varies depending on the needs of a client. Some clients may only need short term support while transferring back home from a hospitalization, while others may require more prolonged support to manage chronic health care conditions and safely remain at home.

How involved do family members need to be in the process?
Because our program focuses on creating highly individualized plans of care, family input and feedback can be very useful during the assessment phase. Following this phase family members can choose their level of involvement in care. Some of our family members attend weekly meetings, while others live out-of-town and receive updates by phone or email.

What is the difference between your fee-for services program, and other community programs?
Many public sector care management programs are limited in the services that they may provide due to large case loads, high turn over in staff, or funding limitations. The Hebrew Home at Riverdale offers a complete continuum of care so that our geriatric care management clients have access to variety of services that address their fluctuating needs. This unique feature sets our geriatric care management team apart from other community based programs. Our services are also available on a 24/7 basis to support both clients and family members during an emergency.

Why would I need to hire someone to coordinate my parent’s care when I can research options on the internet myself?
The final stages of older adults lives can leave them vulnerable, especially if you are providing care from out-of-state. The responsibility of making difficult decisions related to their care can be overwhelming. Our staff has expertise knowledge regarding the needs of older adults, and has access to a team of top professionals that can be brought into the comfort of a client’s home.
