The ElderServe Long-Term Home Health Care Program is designed for persons who choose to remain in their own home although are eligible for placement in a long term-care facility. The program provides a full range of health care, social and supportive services, including medical management, nursing services, rehabilitation therapies and personal care in a client’s own home. Additional care options include home maintenance, Adult Day services and a personal emergency response system.
A case manager, who is a registered nurse, plans, supervises and coordinates all Long-Term Home Health Care services. When a referral is made, a nurse visits the home to conduct a comprehensive assessment of the client’s needs and then develops a plan of care in conjunction with the client, the client’s physician and family Acting as a liaison with the client’s physician and all other ElderServe community programs, the case manager responds to all client requests, questions and concerns.
