As part of The Living Well Journey, we are always striving to enhance and enable communication between the Home and family members.
As a means toward this goal, we would like to remind you that we have Neighborhood Information Cards available to families. This card provides space for contact information for all members of your relative’s care team.
Please take one from the Family Neighborhood News board and consult your Resident Services Coordinator or Clinical Nurse Manager for assistance in filling it out. |
