Appropriate for those with chronic illnesses or those who qualify for and require long term care but wish to stay in their own home. In addition to the many health care options, we can help coordinate social model adult day services, home maintenance, home improvement and moving.

ElderServe Licensed Home Care Services

Appropriate for those wish to remain in their homes but have physical or cognitive limitations and need assistance with daily living. Hourly and live-in services available.

Description of Services:
One-on-one health care or home services from light housekeeping, shopping, escorts to appointments, laundry, meal preparation, personal care, companionship, medication reminders, etc.

Private pay, Contracts with Certified Home Health Agencies and Long-Term Home Health Care Agencies, contracts with government agencies, such as the Office for Aging, long-term insurance.

Telemedicine

Ideal for those who need daily or weekly monitoring of vital statistics (e.g. hypertension patients) but cannot or do not wish to travel to do so.

Description of Services:
• In-home monitoring device that checks BP, weight, heart rate and oxygen levels in blood.
• Information is evaluated daily by an RN who interfaces with individual doctors as appropriate to provide timely action and/or medication adjustment.

Safe Center for Seniors and Social Adult Day Clearinghouse

Appropriate for seniors and caregivers who are searching for resources related to elder care.

Description of Services:
The service offers information, care assistance, counseling, assessment, resource choices and links to many services in the community.

Elder Abuse Services

Elderly victims of neglect, or physical, sexual or financial abuse.

Description of Services:
Victims are removed from imminent danger and a plan of care is developed. Following their stay, the clients return home - if safe - with community support services and oversight by ElderServe; transfer to an apartment in RiverWalk, The Hebrew Home’s senior housing community, or are admitted to The Hebrew Home’s long-term care facility.

If legal assistance is required, the Pace Women’s Justice Center provides legal support.
